Fractional laser: ablative or not, the trials came out level
Pooled trials found no difference between ablative and non-ablative fractional lasers — on results or on pigmentation risk. Downtime is what changes.

Is ablative or non-ablative fractional laser better for acne scars?
Fractional lasers deliver energy in columns, leaving untreated skin between them to speed healing. The pooled evidence does not support the assumption that the more aggressive option works better: a meta-analysis of eight studies found no significant difference between fractional CO2 and non-CO2 lasers on clinician assessment, patient assessment, or the incidence of post-inflammatory hyperpigmentation. What separates them is recovery time and pain.

What "fractional" means
Not a device — a way of delivering energy. Instead of treating the whole surface, the laser places microscopic columns of injury with untreated skin between them. The intact skin around each column drives the repair, which is why fractional treatment recovers faster than treating everything at once.
Within that, two families:
- Ablative — CO2 and Er:YAG. Tissue in each column is removed. More recovery.
- Non-ablative — the columns are heated but not removed. Less recovery.
In South Korea both are sold under a crowd of platform names. What you need is which family, and at what settings.
The finding that should change how you choose
The intuition is that ablative is the serious version and non-ablative is the light one. The pooled evidence does not support it.
A meta-analysis of eight studies compared fractional CO2 with non-CO2 laser therapies for acne scars and found no significant difference on:
- Clinical improvement
- Observer assessment — p = 0.19
- Patient assessment — p = 0.91
- Post-inflammatory hyperpigmentation — p = 0.69
The reviewers' summary is that the efficacy of fractional CO2 "appeared to be equal to that of non-CO2 laser therapies", with a call for better-designed trials.
Two honest caveats. Eight studies is not a large body of work, and the reviewers say so. And "no significant difference" in a small pooled analysis means a difference was not detected, not that one cannot exist.
But it does mean the aggressive option is not carrying evidence the gentler one lacks — including on the pigmentation risk that matters most in Asian skin.
What the risk actually looks like in Asian skin
Numbers rather than warnings.
A 2025 propensity-score-adjusted study of 397 patients with Fitzpatrick skin types III–IV found post-inflammatory hyperpigmentation in 5.42% treated with fractional laser and 4.71% with fractional radiofrequency — no significant difference. Post-treatment scarring was under 1% for the laser group. No infections.
That is a genuine risk worth a conversation, and it is not the reason to avoid the treatment.
Against RF microneedling
The same study is the useful head-to-head, because both are offered for the same problem.
At six months, 88.1% of the radiofrequency group and 71.9% of the laser group reached at least 25% improvement (p = 0.031), with mean improvement of 54.9% against 47.5% — a difference that did not reach significance (p = 0.059). Radiofrequency was significantly more painful: 5.65 against 4.14 out of 10.
The authors conclude that both are effective and well tolerated in this population. So this is a choice between two workable options, not a right and a wrong one.
What should actually decide it
Since the outcome data do not separate the options, decide on the things that do differ:
- Downtime. Ablative needs more. If you are here for eight days, that is a real constraint.
- Pain. Higher with radiofrequency in the head-to-head; higher with ablative among lasers.
- Your skin type and history. If you pigment easily or have pigmented after a previous procedure, say so before anything else.
- The operator. Across every evidence base on this site, settings and experience move outcomes more than the brand does.

- What is the plan if I hyperpigment afterwards — and can you treat it before I fly home?

The bottom line
Fractional lasers work in columns, and the pooled trials found fractional CO2 and non-ablative lasers equal on clinician assessment, patient assessment and pigmentation risk — so the more aggressive option is not buying a better result in the data. In Fitzpatrick III–IV skin, post-inflammatory hyperpigmentation ran around 5% for both fractional laser and fractional radiofrequency. Choose on downtime, pain, and the operator, and ask what happens if you pigment before you leave the country.
