What a collagen stimulator is

Hyaluronic acid filler works by being there. Inject it, and the gel occupies space; the change is immediate and it lasts as long as the gel does.

A collagen stimulator works differently. The injected material is not the point — the body's reaction to it is. Poly-D,L-lactic acid (PDLLA) is a biodegradable polymer that provokes fibroblasts to lay down new collagen over the following months, after which the polymer itself is broken down and gone. What remains, if it works, is your own tissue.

Juvelook is built on PDLLA, in a formulation that also contains non-crosslinked hyaluronic acid.

Three consequences of that mechanism

Everything a patient needs to decide follows from the mechanism, and none of it is obvious from the marketing.

It is slow. Collagen is laid down over months, not days. The trial below measured to week 24. Judging this treatment at two weeks tells you nothing.

Early volume is not the result. Because these products often include hyaluronic acid, there can be a little immediate fullness that then settles as the HA disperses. Mistaking that for the outcome leads people to conclude too early, in either direction.

It cannot simply be dissolved. Hyaluronidase breaks down hyaluronic acid. It does not remove PDLLA. This is the single most important practical difference from HA filler, and it argues for placing less rather than more, particularly the first time.

What the trial found

A 2026 split-face study in Skin Research and Technology compared a PDLLA-based product with PLLA — the class Sculptra belongs to — in 33 subjects, mean age 48.5.

The design is its strength: each person received PDLLA on one nasolabial fold and PLLA on the other, so each participant acted as their own control. Three injections at weeks 0, 4, and 8; assessment out to week 24.

  • Both products significantly reduced wrinkle severity at every timepoint against baseline (p < 0.001).
  • PDLLA improved wrinkle severity by −1.37 at week 24, PLLA by −1.20 — with no statistically significant difference between them on non-inferiority testing.
  • 80% of patients were satisfied or greatly satisfied; 70% had no preference between the two sides of their own face.

That last figure is the most informative in the study. When people cannot tell their own two sides apart, claims that one polymer is meaningfully superior deserve scepticism.

The authors' conclusion is appropriately narrow: "The PDLLA based product demonstrated non-inferiority to PLLA for NLF correction. Further studies are needed to assess long-term safety."

The limits of that evidence

Thirty-three people. One anatomical area. Twenty-four weeks. A well-designed study, but a small one, and the authors themselves flag long-term safety as unsettled.

Collagen stimulators as a class have a known late complication in nodule formation, and "long-term safety" is the phrase doing that work in the conclusion. It is a reason to prefer an experienced injector and a conservative first treatment, not a reason to avoid the class.

Side effects reported

Most subjects had mild swelling for three to four days. One had moderate swelling and tenderness after a second PDLLA injection, which resolved with an intralesional steroid and hyaluronidase.

Worth reading that carefully: the hyaluronidase was acting on the hyaluronic acid component of the product. It does not dissolve the PDLLA.
