Does Sculptra work, and how likely are nodules?
Sculptra is injectable poly-L-lactic acid, which provokes your own collagen over months rather than filling space on the day. The reported durability is genuinely long — at least 25 months in the pooled studies — but the evidence behind that is rated low quality, and nodules are the complication to understand. Published rates run from 1.2% to 41%, and the single variable that moves them most is how much the product was diluted.

What it is, and what it is not
Poly-L-lactic acid microparticles, reconstituted with sterile water into a suspension and injected. Over the following months the particles are broken down and the tissue lays down collagen in response.
So it is a collagen stimulator, not a filler. Nothing is being placed to occupy a space. On the day of treatment you may look slightly fuller from the injected fluid, and that is not the result — it goes, and the real change arrives over three to six months.
Two consequences follow, and they are the reason to read the rest of this page:
- You cannot judge it early.
- You cannot undo it.
What the evidence shows
A 2024 systematic review in Polymers pooled 11 randomised trials covering roughly 1,169 participants.
Four studies found increased dermal thickness and significant improvement in facial lipoatrophy, with effects sustained for at least 25 months. Two found PLLA superior to injectable human collagen; three found it better than polyacrylamide hydrogel on lipoatrophy severity, skin quality, elasticity and satisfaction.
Then the reviewers' own verdict, which is the sentence to carry: PLLA is "a highly effective and long-lasting treatment for facial aesthetics with a reasonable safety profile. However, all findings are supported by low-quality evidence." Five of the eleven studies carried a high risk of bias.
Long-lasting and well liked. Not well proven.
The nodule question, and why the numbers disagree
Nodules are the complication that defines this treatment, and the published rates are wildly inconsistent — which is itself the finding.
- The 2024 systematic review reports nodule rates across studies of 6% to 41%.
- A 2026 scoping review of 40 studies puts incidence at 1.2% to 14%.
The second review explains the gap, and the explanation is useful: dilution volume. Early practice reconstituted a vial in 2–3 mL, and those studies had much higher nodule rates. At 8–10 mL or more per vial — now typical for facial treatment — the rate falls to roughly 1.3–4%.
Two more details worth having before a consultation:
Timing. Nodules appear in two patterns — early at 4–8 weeks from particle aggregation, or delayed beyond six months, most commonly at 9–14 months and sometimes past two years. A good result at three months does not mean you are clear.
Site. The scoping review names the periorbital region, temples and nasolabial folds as higher risk, requiring careful technique.
Nothing dissolves it
Hyaluronidase breaks down hyaluronic acid. It does nothing to poly-L-lactic acid.
That is why the scoping review's conclusion is about prevention rather than treatment: "Prevention through adequate dilution and correct injection technique remains the most evidence-supported approach."
For a patient, this converts into a simple rule. Start conservatively. A course can always be extended; 25 months of a result you dislike cannot be shortened.
What to ask at a consultation
- How many millilitres are you reconstituting each vial with? This is the single question most connected to the published nodule rates, and a clinic that does this often will answer it without hesitation.
- How many sessions, at what interval, and what is the total cost? It is a course.
- When should I judge the result? The answer should be months, not weeks.
- What is the plan if a nodule appears at nine months? Ask before, not after.
- Which product, in Korean? Several PLLA products are sold in South Korea beyond the original brand. Check the licence.
The bottom line
Sculptra builds volume through your own collagen over months and the improvement is sustained for at least 25 months — but the reviewers who found that rate the whole evidence base as low quality. Nodules are the real trade-off: reported at 1.2–14% in one review and 6–41% in another, with dilution volume the variable that separates them, and with onset sometimes past a year. Nothing dissolves it, so ask how much diluent is being used, start conservatively, and judge late.

- When would a nodule appear?
- Two patterns. Early, at 4 to 8 weeks, from particle aggregation. Or delayed, after six months, most commonly between 9 and 14 months, and occasionally beyond two years. So a smooth result at three months is not proof you are past it.
- Can Sculptra be dissolved?
- No. Hyaluronidase dissolves hyaluronic acid and has no effect on poly-L-lactic acid. The scoping review's own conclusion is that prevention through adequate dilution and correct technique is the most evidence-supported approach — because management after the fact is harder.
- Sculptra or ordinary filler?
- They answer different questions. Filler places volume where it is injected, works immediately, and can be dissolved. Sculptra provokes a diffuse collagen response over months, lasts far longer, and cannot be undone. For a defined hollow, filler. For general facial volume loss over a broad area, this is the category — accepting that you are committing.
