Thermage is a monopolar radiofrequency device: it passes current through tissue so the tissue's own resistance heats it, contracting collagen immediately and prompting new collagen over the following months. What monopolar radiofrequency is
Radiofrequency treatment does not heat the skin from outside. It passes an electrical current through tissue, and the tissue's own resistance turns that current into heat from within. Heated collagen contracts immediately, and the controlled injury prompts the body to lay down new collagen over the following months.
Thermage is the monopolar version: one active electrode on the skin, with the current returning through a grounding pad elsewhere on the body. That arrangement is what produces its defining characteristic — energy spread through a large volume of tissue, deep.
The three arrangements, and why the difference matters
A Korean clinic will offer you radiofrequency under half a dozen device names without ever mentioning which arrangement each uses. The arrangement is the useful information.
Monopolar reaches deepest — up to around 20 mm according to the 2025 review cited here — with diffuse energy distribution, and is the most painful. Bipolar confines the energy between two electrodes at 1–4 mm, which is shallower and more controlled. Microneedle radiofrequency puts the energy through needles, so it is delivered past the epidermis to a depth the operator sets.

The 2025 review in Health Science Reports gathers what has been published. Its headline figures for monopolar devices:
- A multicentre trial reporting 83.2% periorbital improvement and eyebrow lifting greater than 0.5 mm in 61.5% of 86 patients.
- Satisfaction across studies ranging from 50% to 82%.
- 73.5% observing improvement within two months.
That satisfaction range is worth pausing on. A spread from half to four-fifths of patients is not a rounding difference; it reflects genuinely different results across studies, devices, and operators. Any single satisfaction figure quoted to you is a selection from that range.
The reviewers' own conclusion is the sentence to take away: efficacy "depends critically on the customization of treatment parameters and the expertise of the practitioner."
Pain, cooling, and what can go wrong
Depth costs comfort. Monopolar is the most uncomfortable of the three arrangements, precisely because it is heating a large volume.
It also carries the clearest hazard, which is burning. These devices apply surface cooling to protect the epidermis while the deeper tissue heats, and the review notes the trade-off explicitly: more cooling protects the surface but works against the depth you were paying for. Getting that balance right is a setting the operator chooses, which is the practical meaning of "expertise of the practitioner".
What it does not do
- It does not reposition tissue. Descended tissue is a thread lift or surgical question.
- It does not add volume. A hollow is a filler question.
- It is not immediate in its final form. There is a contraction effect on the day, but the substantive change accrues over months, and judging it at two weeks is judging it too early.
