What Ultherapy is
Ultherapy delivers microfocused ultrasound — ultrasound energy focused to a point below the skin surface, hot enough at that point to create a small zone of thermal injury, which the body then repairs. The repair is the treatment: new collagen laid down over the following months, and tissue that behaves slightly tighter as a result.
The "with visualization" part matters and is what the V in MFU-V stands for. The device images the tissue before it fires, so the operator can see the layers they are treating rather than working blind.
Why the depth is the whole argument
Focused ultrasound is delivered at fixed depths. The shallow settings work in the dermis. The deepest — around 4.5 mm — reaches the SMAS, the fibrous layer a surgeon pulls on during a facelift.
That is the entire case for the technology, and it is also the case's limit. Reaching the SMAS with heat is not the same as pulling on it with sutures. The effect is a tightening of tissue that is already where it should be, not a relocation of tissue that has fallen.
What the evidence shows
A 2024 systematic review and meta-analysis in the Aesthetic Surgery Journal screened 4,019 references and pooled 42 studies. Its findings, with the numbers as published:
- Investigator-assessed improvement: 89% (95% CI 81–94%)
- Patient-reported improvement: 84% (95% CI 73–91%)
- Satisfaction: 84% overall, 93% for the face and neck
- Mean pain: 4.85 out of 10
- Adverse events — redness, swelling, oedema, bruising, tenderness — mild to moderate, with no serious adverse events or long-term sequelae reported
One figure in that list deserves separating out, because it says something about how satisfaction data in aesthetics is produced. Pooled satisfaction was 84% — but 62% in the studies that gave patients a neutral option rather than making them choose satisfied or dissatisfied. The same treatment scores twenty points differently depending on how the question is asked. When a clinic quotes you a satisfaction percentage, that is the question to ask about it.
The authors' own summary is measured: an enhancement in aesthetic score and satisfaction, a moderate pain score, and a generally favourable safety profile. Not a facelift, and not nothing.
Shurink, Ultraformer, and the Korean devices
A visitor to a Korean clinic will more often be offered Shurink (슈링크), Ultraformer (울트라포머), Doublo (더블로) or Liftera than Ultherapy itself. These are focused-ultrasound devices in the same category, usually at a lower price per session.
Two honest points about that. First, the evidence above is for microfocused ultrasound as a class — it does not establish that any one of these performs like any other, and a clinic asserting a specific brand advantage should be able to say on what basis. Second, the difference you can actually verify is regulatory: ask for the device's Korean product name and look up its licence.
Side effects and who does it
Reported effects are redness, swelling, and tenderness, mild to moderate and short-lived. The meta-analysis reported no serious adverse events across its 42 studies.
The caveat is operator-dependent. Focused ultrasound is delivered at chosen depths and energy levels, in chosen positions, and getting those wrong is how the uncommon problems — fat loss where it was not wanted, nerve irritation — happen. Who holds the handpiece matters more than which logo is on the machine.
What it does not do
- It does not reposition tissue. Descended tissue is a thread lift or surgery question.
- It does not add volume. A hollow is a filler question.
- It does not resurface skin. Texture and pigment are laser questions.
- It does not work on the day. The change develops over months, which is also why judging it too early is a common disappointment.

- Which device, and what is its Korean product name? Look up its licence.
- How many shots, at which depths, and over which areas?
- Who operates it — and are they a doctor?
- What are you expecting this to change, specifically, and when should I judge it?
- If you are also proposing a thread lift or filler, what is each one for?

- It will not reposition tissue that has descended — that is what a thread lift or surgery does — and it will not add volume where volume is missing. If a clinic is offering it for a hollow, the tool does not match the problem.
